"That's when everything changes"

That's what River told the Doctor at the end of "The Big Bang", when he found out who she was everything would change.  Other than a timey-wimey surprise as to her identity, what else do you think has changed for him?  I remember the speculation that "everything" had to mean something pretty epic - could she have meant his "death" and no longer being such a visible presence in the universe?

Didn't she also say it to him in the Angels episode of S5? I think more often than not that when she says these things it's just at the whim of Moff as a way to keep us hooked in, but in the long run I don't see much actually changing, except for the personal relationships of River with those around her. I suppose the wedding, or at least ceremony, was a big change for both of them and that could have repurcussions but I suppose the biggest change is that he is now essentially in hiding as a result of everything thats happened.
