Why wasn't Donna swept away by the flood, as she was on the lowest level?

Also, when they went back to the formation of the Earth, the Doctor says he's never been back that far, but he did in The Edge of Destruction.

This is a bit picky, but there is a scene where the TARDIS door is open, then closed, then open again without anyone touching it.
