Several of the Time Lords die, but don't regenerate. Perhaps more of an inconsistency than a blooper, but it stood out to me.

There's actually no sign of regenerating after being killed in any of the Gallifrey-based stories.

In the first episode the Master kills one of the guards but then he is alive again in the next episode. At least, I'm 99% certain it's the same guy.

There was a certain appropriateness about that post being duplicated I suspect this may have been a deliberate mistake, to show the unreality in the Matrix, but just before the Doctor's foot gets caught in the points at the end of episode 2, we're clearly shown that he's standing with his foot outside it. There's also an inconsistency from this episode, rather than in it, but I only noticed it on this watch, so I thought I'd mention it. The Master was wearing the Sash of Rassilon when he fell into the abyss, yet it was used for the presidential induction in The Invasion of Time. Of course, the Master escaped from the abyss, but I doubt he'd have gone out of his way to return it to its rightful owners.

The number of floors the building has is 53 in the beginning, but then said to be 52 later on.
