When the Doctor ripped the strap off the ceiling of the ship, it was actually a blooper they decided to keep in.

Time of Angels was apparantly a working title for Gridlock. I don't see how that would have fit though.

No, but it's interesting.  I can sort of just see a connection with Gridlock, but I don't think it would have been a good title for that, whereas it's a brilliant title for this one.

Is that counting the Doctor as one or three?  Maybe I'm just tired, but the only others I can think of are Rose and the Master.

Not solo.  She took part in the joint effort in Journey's End, but she didn't fly it by herself.

The only person (other than the Doctor) I can think of who flew it solo in the classic series was Adric in Full Circle, though Nyssa and Tegan did as a joint effort in Castrovalva.
