There's no way a planet could achieve a mediaeval-level civilisation if the entire planet had one city and "a few settlements in the north". A country on that planet might just manage it, if there's plenty of other countries to trade with.

We get an example of this in nearly every story. It always makes me laugh And, of course, the way the baddies march determinedly past inadequate hiding places with eyes fixed straight ahead, when they'd see the Doctor if they so much as glanced aside.
