I'm not sure about that.  In The Rebel Flesh, Buzzer is fully aware of his Flesh body having been knocked into the acid, and I always understood that while they were in the harnesses, they actually were the Flesh body.  In the same way, Amy's asleep and gets occasional glimpses of Kovarian looking at her, so I always imagined she actually experienced everything the body experienced.

I was never completely clear on how that worked, especially in the second part where the Flesh versions rebelled and were acting independently from their original versions.  I don't recall that each knew what the other was doing or thinking when they declared war on their counterparts.

No, that was different, and was meant to be impossible (like the Titanic sinking was impossible Cool).  When the Doctor destroys Amy's Flesh body, her real self wakes up, so that implies it's the same state as when the others are in harness - they refer to it as being "in their ganger".
