Gallifrey

Gallifrey is the home planet of the Time Lords.  Physical features that are frequently mentioned are skies of burnt orange, trees with silver leaves and red grass.  The main building is the Capitol, from which the High Council rule and where all major events take place.  Gallifrey was destroyed in the Time War, but remains intact within a time lock.  An attempt by the High Council to break out of the time lock was thwarted by the Doctor.

Gallifrey was first alluded to in the very first episode and described by Susan in The Sensorites, without being named.  Similarly unnamed, Gallifrey appeared in The War Games, Colony in Space and The Three Doctors, before being casually given a name in The Time Warrior.  Stories set substantially on Gallifrey since then have included The Deadly Assassin, The Invasion of Time, Arc of Infinity and The Five Doctors.
