A fan rumour was that Hartnells scenes were shot in his garden shed, but this is false.

This was William Hartnells final performance both as the Doctor and as an actor before he passed away in 1975

It was intended to include Jamie and Zoe, but Frazer Hines was busy working on Emmerdale and Jon Pertwee said no to having Zoe as he thought it would mean too many companions

The original script, entitled Deathworld, had the Time Lords enlisting the three Doctors to battle a Federation of Evil led by Death itself.

Wasn't she asked to stay on as his companion originally?

I haven't actually heard that, but possibly. PT and FH both wanted to leave, but she hadn't been around for that long.

I think it's on one of the extras. Perhaps for Spearhead?

Also, I just did a quick look to see if it was anywhere online and there is this interview with Wendy Padbury where she says Frazer was asked to stay too, but I'd never heard that part.
I don't remember anything from my penultimate story "The Space Pirates", but l recently saw the first five episodes of “The War Games”. Watching it was like seeing someone else. Usually if I see a clip I remember things about what I was doing, but I remember nothing of "The War Games" which is quite extraordinary. I remember the people in it, although I don't remember where the locations were, but I enjoyed seeing it again and became quite involved. This was Pat's last story, but we were asked to stay on alongside the next Doctor and Frazer and I decided it would be nicer to go with him. We didn't know who was coming in. I know it turned out to be Jon Pertwee and we'd probably have had a lovely time, but I didn't think you could push your luck.

I think it's on one of the extras. Perhaps for Spearhead? It's not Spearhead, because I watched the extras for that just a few weeks ago. Possibly The War Games. Though I'd heard Frazer Hines had already decided to go and was persuaded to stay and leave at the same time as Troughton. Then again, people never remember things happening in quite the same way, and maybe that's the way she remembered it.
