I came across this somewhere on Facebook - you may have heard it, but the full implications of the theory were new to me:

In The Pandorica Opens, River buys a vortex manipulator from Dorium, which he describes as taken off the wrist of a time agent.

Dorium worked frequently with the Headless Monks, who decapitate their victims.

Jack Harkness, who was a time agent and has a V.M., is believed to have ended up as a disembodied head.
